Question Please Help me applying Passport

Hi,
I did my College(2005-2009) in Tirunleveli. Now i am in Chennai.(Tirunelveli and Chennai are two cities in Tamil Nadu). I am living in Chennai in my home from my birth. My permanent address also the same. While Applying passport, there is one Column, that if we didn't stay continuously for the past one year, we have to mention the past address. For past 4 years I stayed in Tirunelveli, College Hostel. SO should i mention that. Now i am staying in my permanent address that is my Home in Chennai. If i mention my hostel address, then should i submit any proof. I have bonafide certificate, but it was expired. It was dated upto May 2009, since my college is over on May 2009. What should i do? Should i mention my hostel Address?

My Friends are saying that Since i am living in my Permanent address, we need not mention hostel address. What should i do?

Another Doubt,
I opened SBI account during FEB 09 only. Can i submit it as address proof along with Ration card(They are asking two proof). Some are saying, bank account should be one year old. And some are saying for Takkal only it should be one year old, for normal its not necessary. What shall i do? i don't have another proof.
